You may be eligible for a Home Medicines Review, conducted by our accredited consultant pharmacist!
What are some of the benefits of having a Home Medicines Review?
- Provides an opportunity to have your medicines assessed to optimise your treatment;
- You can discuss your medicines with a fully qualified consultant pharmacist, to improve your understanding of your medicines and get the best possible response;
- The pharmacist will provide a report to your doctor who will discuss the findings with you at a follow up appointment.
How much will a Home Medicines Review cost me?
A Home Medicines Review and the follow up GP consultation are fully funded by Medicare – meaning no out of pocket expense to you!
How often can I have a Home Medicines review?
A Home Medicines Review can be conducted every 2 years or as often as required, if you have significant changes to your medication or health (e.g. recently discharged from hospital).
