If you are over 75, you can have a FREE annual health assessment.
What are some of the benefits of a 75+ Health Assessment?
- A trained nurse will assess your general health needs to help you stay healthy and independent in your own home.
- It can help you access subsidised services to stay in your own home. E.g. cleaning services.
How often can I have a Health Assessment?
A 75+ Health Assessment can be conducted every 12 months.
How much will a Health Assessment cost me?
A 75+ Health Assessment is fully funded by Medicare – meaning no out of pocket expense to you!
Where does the assessment take place?
Although it is preferable that our practice nurse comes to your home, it is also possible to conduct the assessment at the surgery if you would rather (please advise reception staff or practice nurse at the time of booking).
